Eventide provides sourcing, technology, telemedicine, and partnership services to aesthetic and wellness practices. Our intake forms allow providers to request information about our products and services. We are a business‑to‑business partner and supplier; we are not the treating provider.
Our services and communications are intended for licensed professionals and businesses. By contacting us, you represent that you are at least 18 years old and authorized to request information on behalf of your practice.
Text messaging is optional and requires affirmative opt‑in. We send text messages only to individuals who provide a phone number and check the SMS consent box on our form (unchecked by default). When you opt in, you consent to receive recurring automated text messages from Eventide Aesthetics & Wellness, including promotions, special offers, new product & device announcements, educational updates, a message confirming your inquiry, an appointment‑scheduling link, and follow‑up messages from our team regarding your request. Consent is not a condition of any purchase.
Information we provide is for general business and educational purposes for licensed providers and is not medical advice, a diagnosis, or a treatment recommendation, and is not a guarantee of any outcome. All treatments and prescriptions are determined and delivered by licensed providers based on individual evaluation. Compounded medications and biologic or regenerative products are not FDA‑approved; availability and permitted use vary by state.
